Earlier this year, Dr. Sylvan. Bartlett, a double board certified plastic surgeon who has performed over 5,500 surgeries began airing a series of Breast Augmentation radio interviews. Per to the FDA website, “breast augmentation patients may need to have one or more re-operations over the course of their life due to one complication or a combination of local complications”. One of the most common breast augmentation complications is called Capsular Contraction, a painful condition that is caused when scar tissue forms around the implant. The scar tissue squeezes around the implant and feels unnaturally hard to the touch and in the case of Grades III and IV Capsular Contracture, could be disfiguring.

Sylvan Bartlett, M.D., P.A., says “As far as batting averages are concerned, I have a very low rate of Capsular Contracture. To the best of my knowledge, I haven’t had to re-operate a patient on whom I had performed a simple breast augmentation to correct Capsular Contractions in 9 years or more.”

The interviews also explain why Dr. Bartlett pioneered a 1983 research study to determine the correlation between bacteria that exited the nipple and Capsular Contraction. During the span of an entire year, prior to the surgery he prepped the patient’s breast, covered them with a sterile dressing, then approximately an hour later cultured the nipples adjacent skin as a control on each breast augmentation patient. The research was conclusive: nearly every single breast augmentation patient test positive developed Capsular Contracture. He first tried to decrease the bacterial count in the ducts by giving antibiotics that the bacteria was sensitive to. This dropped the capsular contracture rate by one third of what it had been but patients continued to have fiberous contracture, moreover, some patients developed allergies to the antibiotics. Dr. Bartlett later decided to cover the nipples with a sterile membrane as soon as the breasts were prepped and draped. This avoided possible possible contamination from microscopic amounts of bacteria that could occur during surgery. This also eliminated the course of antibiotics. In the presence of a foreign body the bacteria can attach itself to the foreign body and then it can cover itself with a mucopolysaccharide (biofilm), which makes it impossible to eradicate unless the foreign body is removed. Today, more and more plastic surgeons are adopting this Capsular Contraction mitigating surgical technique and reporting less incidence of complications.

“There’s an absurd amount of marketing hype surrounding breast augmentation,” says Dr. Bartlett, “People tend to hype what works in their hand and run things down that do not give them good results. This does not mean that the same thing that don’t work with their approach will not work with a surgeon who uses a different approach. My hope is that those listening to my radio interviews will become better informed and less confused by the barrage of plastic surgery marketing bias found on the internet. At the end of the day, surgery is a big decision, and I consider myself a true patient advocate.”
